Flatbacks and threadless systems are common in professional piercing, but they are components—not a promise that one style suits every piercing. Start by understanding the post, the decorative end and the complete piece.
End, post and complete piece
Flatback / labret post
The wearable post passes through the piercing and finishes in a flat disc at the back. Its gauge and length need to suit the placement and wearer.
Decorative end
The visible design at the front. In a threadless system, it has a small pin that fits into a compatible hollow post.
Complete piece
A compatible decorative end combined with the correctly sized post creates the wearable flatback jewellery.

How threadless jewellery attaches
The APP describes threadless jewellery as a press-fit system using a pin coupling on the ornamental end, with no screw threads. The pin is fitted into a compatible post and held by tension.
Compatibility matters. The post gauge, internal dimensions, end size and manufacturer tolerances all affect how securely components fit.
Internal threading vs external threading
With internally threaded jewellery, the wearable shaft that passes through tissue is smooth and the removable end carries the exposed screw thread. The APP includes internally threaded jewellery within its standard for initial piercing jewellery.
Externally threaded jewellery has the screw pattern cut into the post itself, meaning the rough thread can pass through tissue during insertion or removal. APP guidance distinguishes this from internal threading and threadless press-fit systems.
Secure fit, correct length and the right timing
Flatbacks and threadless systems can be excellent choices in suitable placements, but they are not automatically the right choice for every piercing. Post length, gauge, end size, anatomy and healing stage still need to be considered.
APP aftercare guidance recommends checking threaded and threadless ends regularly with clean hands or paper products, and having jewellery changes during healing performed by a qualified piercer.
